Types of Fibroids

Fibroids are defined according to where they occur in the uterus:

• Intramural fibroids occur within the muscular uterine wall
• Submucosal fibroids are present just beneath the uterine lining
• Subserosal fibroids are on the outside of the uterine wall

Diagnosis

• To confirm if a patient needs to undergo a myomectomy, doctors conduct a full pelvic floor examination, assessing if the uterus is enlarged or irregularly shaped.
• They do an ultrasound which gives them insight into the number, location, and size of the fibroids.
• At times, patients are also advised to undergo an MRI so that the surgeon gains a more detailed understanding of how severe the condition is.
• Doctors may also suggest conducting a Hyster sonography, i.e., a saline infused sonogram which provides clear images of the uterus and fibroids or a hysteroscopy, i.e., a minimally invasive technique where a thin, lighted tube is inserted into the uterus to examine fibroids.

Treatment

• An abdominal myomectomy is also known as an open myomectomy and is recommended for those patients with large or multiple fibroids. It is performed via a large incision in the lower abdomen
• A laparoscopic myomectomy is a minimally invasive procedure which involves small incisions and the use of a camera to guide the removal of fibroids
• A hysteroscopic myomectomy is performed when the fibroids are inside the uterine cavity. Instruments are inserted through the vagina and cervix without external incisions

Pre surgery care

Based on the symptoms, physical examination, imaging results and the patient’s future pregnancy plans, doctors decide what type of myomectomy to perform. To prep a patient for surgery doctors might prescribe a gonadotropin-releasing hormone (GnRH) agonist to shrink the fibroids.

Patients are advised to stop their consumption of alcohol, caffeine and heavy foods and are required to fast for a specific period before the surgery.

Any blood thinners and extra medications are discontinued. Once the patient has been prepped properly, they undergo the surgery.

Post-operative care

Post operative care is crucial for a smooth recovery and minimal complications. Patients are often prescribed pain medications to ease their discomfort. They are monitored for complications such as infections, excessive bleeding or unusual pain. They are advised to hydrate well and are encouraged to walk short distances to prevent blood clots and improve circulation. While light walking is encouraged, lifting heavy objects is to be avoided for at least four to six weeks.

Recovery

Patients are counselled to avoid stressful situations and get adequate rest. This is especially important because getting a myomectomy can have a massive psychological impact on the emotional wellbeing of a woman.

Post surgery, bloating and surgical scars can adversely affect self-esteem and confidence. Those who hoped the surgery would improve their chances of reproduction often fear that their fibroids will reoccur. Therefore, mental wellbeing also needs to be prioritised.

In addition, patients are advised to consume a high fibre diet which will prevent constipation, iron rich foods for those who have experienced heavy blood loss before or during the surgery, and protein to aid in tissue and muscle repair. They are strongly discouraged to consume sugary, fried, and processed foods which can cause inflammation and impede their healing.
